Originally created in 1913 to reward doctors and health workers involved in the fight against the cholera epidemic during the Second Balkan War, the "Sanitary Merit" cross also acquires a military nature, by adding in 1917 the war insignia, the wreath made of oak and laurel leaves, which will in turn disappear after the reorganization of the decoration in 1938. The obverse of the medal features a red enamelled cross surrounded by a series of rays and with the bust of Queen Elizabeth centrally located and facing to the right, while the reverse illustrates the phrase "Sanitary Merit", along with king Carol I's cipher and the year of its foundation. The ribbon made of white mohair with red stripes also has the specific military symbol, the oval wreath of oak and laurel.
