On the obverse: in the foreground of the medal, in five medallions, are represented the effigies of the kings leading the countries participating in the war of 1913, namely: centrally, in a larger medallion, of: 3.30 x 2.60 cm, King Carol I is depicted, the patron of peace, and on either side of him, symmetrically positioned, are four other smaller medallions (of: 2.30 x 1.80 cm), surrounded by olive and oak branches, in which are represented: the king of Serbia, Petar I (top left), the king of Montenegro, Nicholas I (bottom left); the King of Bulgaria, Ferdinand I (top right) and the king of Greece, Constantine XII (bottom right). Above the effigies it writes on three lines, in capitals, of different sizes: "TRATATUL DE PACE/ DE LA/ BUCUREȘTI"; and at the bottom, also on three lines, it writes the date of peace, both in the old style and in the new style, namely: "DIN/ 28 IULIE/ 10 AUGUST/ 1913". The described iconography is separated from the rest of the medal by a frame, on which are represented - opposite each effigy - other small medallions with the initials of the respective kings, but also with the royal coat of arms of the respective country. All these are linked together by a chain thread stylized to be indestructible, which wants to suggest, in fact, the permanence of the concluded peace. Beyond this, at the bottom of the medal, in the left dial is written the name of the author: "C. Alessandrescu"; and in the right one are inscribed the initials: "M.F. AL S.N.R.". Finally, the entire described ensemble is delimited from the edge of the medal by another outline, consisting of small lines and dots.
On the reverse of the medal, in the centre, a 7.30 cm medallion is depicted, marked from the rest by a dotted edge and which has depicted in the foreground Peace, embodied by a woman who holds in her left hand, down, a parchment with the inscription "PAX" ("Peace"), and to the right raises a bunch of olive leaves, as a symbol of victory, while she is stepping on a cannon barrel, from under which other branches of olive and oak are rising, symbols of the same victory. On the background of the medallion, to the left, is represented the building of the Ministry of Foreign Affairs in Bucharest, on a solar background, which covers the entire upper semicircle of the medal, above which it writes in Latin: "TRANQUILLITATIS" (left) and "EUROPAE" (right), ie: "EUROPEAN PEACE".
Beyond the described medallion, on the free portion (of 8 mm) to the edge of the medal, in five dials delimited by plant elements, are inscribed the names of the members of each delegation present at the Peace Conference in Bucharest, in capital letters (of 2 x 1.5 mm) and on three rows. Starting from the top and to the right, those inscribed are:
* "T. Maiorescu - Al. Marghiloman/ Take Ionescu - C.G. Dissescu/ General Coandă - Col. C. Cristescu" - for Romania;
* "D. Tontcheff - General Fitcheff/ Dr. S. Ivantchoff – S. Radeff/ Lt. Colonel Stancioff" - for Bulgaria;
* "E.K. Veniselos – D. Panas/ N. Politis - Cap. A. Exadactylos/ Capitaine C. Pali" - for Greece;
* "General Serdar I. Voukotitch/ I. Matanovitch" - for Montenegro;
* “Nik. P. Pachitch - M.G. Ristitch/ M. Spalaikovitch - Col. K. Smilianitch/ Lt. Colonel F. Kalavatovitch” - for Serbia.
